Optional and Additional Tests that are Available:

  • Coronary Artery Calcium Score/CT Scan: To look for coronary calcium in the coronary (heart) arteries – providing a number or score to demonstrate the extent of disease and location of the disease (which heart arteries contain the most calcium).
  • Sleep Testing: Sleep monitoring (both home and sleep lab options available) may be recommended (based on history, sleep questionnaire and FitBit findings of sleep concerns.   The sleep studies are designed to evaluate the quality of your sleep and identify the cause of sleep issues – such as breathing problems, hypoxia, sleep apnea, or mechanical obstruction during sleep.
  • Stress Testing: A typical stress test (even stress ECHO and stress Nuclear – treadmill or pharmacologic) can miss significant and dangerous disease in some people.  It is far too common for someone to have a stress test and then have a heart attack a short time afterwards.  So, we recommend looking for dangerous disease in other ways.  There are times and situations, however, in which stress tests are very useful.  If we recommend a stress test we most often want a metabolic or cardiopulmonary exercise test.

The Cardiopulmonary Exercise Test is a highly sensitive, non-invasive stress test that assesses how well the heart, lungs, and muscles are working individually, and how these systems are working in unison. Your heart and lungs work together to deliver oxygen to your muscles, where it is used to make energy, and to remove carbon dioxide from your body.

The full cardiopulmonary system is assessed during a CPET by measuring the amount of oxygen your body is using, the amount of carbon dioxide it is producing, your breathing pattern, and electrocardiogram (EKG) while you are riding a stationary bicycle.

The traditional treadmill stress test only relies on the EKG, which only partially assesses the heart and nothing else. Besides detecting problems in multiple body systems, the CPET is also used to monitor changes in your disease condition, the effect of certain medications on your body, and if medical therapy is improving your condition.

  • ECHO or echocardiogram: A sonogram of the heart to evaluate the heart structurally (valves function, muscle function, etc.)
  • Aortagram: A sonogram of the aorta to look for aortic aneurysm and calcification or disease
  • Anke-Brachial Index: A measurement of the pressure in the upper extremities (arms)  and the lower extremities (legs)  – a screening for peripheral arterial disease (PAD)
